Located on Hwy 31 in Pelham, the Community of Hope Health Clinic operates as a primary care facility for those who have limited access to care, other than the Emergency Room. With the exception of a limited paid staff, the providers, nurses, interpreters and others serve as volunteers for this mission!
Are you a prospective patient? All new patients need to gather proofs and call 205-644-8640 to begin the patient intake process. CHHC requires a picture ID, proof of residence (bill), and proof of income (or notarized affidavit stating how you live day-to-day). Once all requirements are met, a prospective patient will be assigned a provider on either Monday or Tuesday mornings OR Thursday nights. Wait time is approximately 1-2 weeks between intake and being seen by a provider.
